Ongoing care that fits around your life

At Dr Joosab Medical Practice, chronic disease management means more than a repeat prescription. Dr Y. Joosab builds up a picture of your diabetes, hypertension or asthma over time, so your care plan reflects how you’re actually doing between visits, not just a single reading on the day.

Whether you’re newly diagnosed or you’ve been managing your condition for years, you’ll see the same doctor at each review, and same-day appointments are available if something changes unexpectedly.

What's included in a chronic disease review

  • Blood pressure, blood glucose or lung function checks appropriate to your condition
  • A review of your current medication and any adjustments your doctor recommends
  • Repeat prescriptions for long-term medication
  • Referral for additional tests or to a specialist, where this is needed
  • Practical guidance on managing your condition day to day

Which conditions are managed at this practice?

Diabetes (Type 1 and Type 2), hypertension and asthma are managed on an ongoing basis, alongside other related long-term conditions. Where a condition needs specialist input, your doctor refers you and continues to coordinate your general care.

How often will I need a review appointment?

This depends on your condition and how well it’s currently controlled. Patients who are stable are often reviewed every one to three months, while those recently diagnosed or adjusting to new medication may be seen more often. Your doctor agrees a schedule with you at your first visit.

What should I bring to my appointment?

  • Your current medication, or a list of what you’re taking
  • Recent test results, if you have them (blood glucose readings, blood pressure log, etc.)
  • Your medical aid card
  • Notes on any new symptoms or changes since your last visit
