What do I need for a medical certificate?

  • A consultation with the doctor, either while you’re currently unwell or for an assessment shortly afterwards
  • Where the certificate is based on your own account of your illness rather than the doctor’s direct observation, this is noted on the certificate, as required by HPCSA rules

What information does the certificate include?

  • The doctor’s name, practice details and qualifications
  • Your name and, where applicable, your employee number
  • The date and time of your examination
  • A description of your illness in plain language, or a statement that you’re unfit for duty, depending on what you consent to disclose
  • Whether you’re unfit for all duties or able to manage lighter duties
  • The recommended period of leave
  • The doctor’s registration number and signature

No. A medical certificate reflects the date you were actually examined and can’t be backdated to cover an earlier absence. If you were unwell but couldn’t get to the practice at the time, mention this to the doctor, who can note the circumstances accordingly during your consultation.

A genuine medical certificate requires an assessment by the doctor. This might be in person or, where clinically appropriate, discussed over the phone, but it can’t be issued without some form of assessment, as this is what makes the certificate valid.

The certificate is prepared to meet the standard information requirements set by the HPCSA, which most employers recognise. If your employer has a specific form, bring it with you and the doctor completes it.
